Q. What should I expect at the initial consultation?

At the initial consultation you will be given an opportunity to discuss your personal health goals with a functional medicine provider. This will be an opportunity for you and our staff to identify if you are a good candidate for our services.

Q. How much is the initial consultation?

The initial consultation is $300 and will be applied to the cost of the program.

Q. Should I bring anything with me to the consultation?

We ask that you bring spouse or significant other, any pertinent medical records, supplements, medication and lab tests results with you.  If you are not the financial decision maker, we ask that you bring your significant other with you to the consultation.

Q. How long should I expect the consultation to last?

Expect to consultation to last for about 60 minutes. If you are a good candidate for our care we will go over the test kits, paperwork, and financing options to get you started. This will be an additional 20-30 minutes. We value your time and do our best to provide a timely service.

Q. Does your office take insurance for functional medicine services?

We do not accept insurance of Medicare for our consulting services.  Your case requires special attention and time that is not covered by insurance companies.  We can utilize health insurance to significantly reduce the out-of-pocket expense for some lab testing.

Q. Does your office offer single lab tests of a-la-carte testing?

We do not offer a-la-carte testing since this goes against the paradigm of functional medicine.  If we are going to address the root cause of your health issues, we will need to take a more comprehensive approach.  We only work with patients on a case management basis in our Holistic Program.

Q. Is my lab testing covered by health insurance?

It depends.  Functional lab testing is usually not covered by most insurance providers.  Pending on your insurance, and the functional lab company, will determine if the cost of the testing is covered or not.  We do our best to find the most affordable solution for the lab testing.  The cost will either be direct provider pricing with no markup or a substantially reduced client responsibility if the lab company bills your insurance.  You will pay the lab company directly not our office.

Q. What forms of payment do you accept?

We accept cash, check, Visa or MasterCard or Discover cards.

Q. Do you offer financing for the program?

Yes, we offer client financing.  Please speak with our office to discuss.

Q. Can I use my HSA or FSA account?

Yes. Please check with your HR department to be sure.

Q. What are your office hours?

Our office hours are by appointment only.  Please see schedule for available days and times.

Q. Do I still need a primary care physician?

Yes, we do not provide acute care services and we do not replace the services that your current medical team provides.  We provide a stand-alone service.

Q. Will I see other practitioners?

Yes.  You will work with a team of experts to help you accomplish your health goals.  These will include the functional medicine provider and a holistic health coach.

Q. Does your office accept patients that live in other cities, states, or countries?

Yes, we can work with any person in any country.  The first consultation must be in office to establish care.  Any further follow care can be done via either phone of a secure HIPPA compliant telemedicine platform.

Q. Do I qualify for the Holistic Program?

This program is for anyone looking to optimize their health. The only time somebody is not qualified is if their health is too far-gone or their condition requires urgent medical attention. If a client is not motivated, engaged and willing to take the steps necessary to get healthy they would not be a good candidate.

Q. How much should I budget for functional lab testing cost?

By joining our Holistic Program you have access to direct pricing on labs test kits, which ensure you pay the lowest price possible.  On average, you should budget between $600-$1200 for functional labs.

Q. Is the Holistic Program covered by health insurance?

Our program service cost is not covered by insurance.

Q. Does the cost of the initial appointment apply toward the cost of the program?

Yes.  Your first appointment is one of your six appointments.  We apply your payment to the total cost of the program and therefor you are paying into the program when you schedule your first appointment.

Q. Can my appointment be done over the phone or Skype?

Yes.  Your first consultation must be done in person so we can meet with you and make sure we’re a good fit for each other.  After the first appointment we will use a secure HIPPA compliant telemedicine platform to conduct follow up consultations.

Q. Who will my second and third appoints be with?

Your second appointment with be with a lifestyle / holistic heath coach and she will be focusing on your dietary aspects, recipes and menu.  She will also provide you with lifestyle coaching and consulting for a total of 6 appointments.  You will have the addition of a lifestyle guidance and monitoring app for your smart phone.  This app is a means of keeping you accountable and connect with your coach and keep you on the right track.  Your third appointment will be with your practitioner who will be overseeing your case, answering your questions and recording your lab result videos.  This third appointment is designed to review your lab results.

Q. Is it mandatory to use all six appointments?

If you are a self-starter and like to be independent it is not mandatory to use all appointments as you have direct clinician access.  The program’s core value is in evaluating your history, building your clinical case, ordering the right testing, educating and designing a unique solution for you that addresses your six pillars of health.  Our goal is to empower you and guide you on the right path so you can use these strategies for the rest of your life. You are always welcome to use the remaining 3 appointments as needed to focus on your clinical, nutritional or coaching needs.

Q. How does the lab testing work?

The lab testing is typically done at home.  In some instances you might have to get blood work done.  We will direct you to the lab that coordinates that or a mobile phlebotomist.  Many of the tests have shipping included in the cost of the kits.  If not, you will be responsible for the shipping.  You will get instructions in more detail on how to do the test kits and the kits come with clear instructions as well.  Payment is made directly to the lab to ensure you have the lowest price possible.

Q. What happens at the end of the 6 months Holistic Program?

Once we have gotten you to a stable place you can follow up as needed.  If you require additional support after your Holistic Program is completed you have the option to pay per visit.  $150 per half hour appointment or $300 per hour.

Q. What is Holistic Hormone Solutions return policy?

Pre-approval is required on all returns.

Refrigerated items cannot be returned.

No supplement returns will be accepted after 30 days on all items.

Special orders can not be returned.

Q. What is the fastest way to order supplements/test kits after I have become a patient?

The quickest and easiest way to order supplements or test kits is during your initial consultation.  Ongoing supplements and refills can be completed through our virtual supplement pharmacy FullScript.  Please see the website for direct link.


Q. What is PA Smith’s background?

Josiah Smith is a physician assistant with both medical undergraduate and graduate degrees.  He also has post graduate education in the study of functional medicine from Functional Medicine University, The Kresser Institute, Holistic Nutrition Lab and has studied extensively under Dr. Kalish and trained in the Kalish Method.  During 2017 Josiah will have completed additional training from the Institute of Functional Medicine for AFMCP (Applying Functional Medicine in Clinical Practice) and trained be a certified functional and lifestyle medicine clinic form the Living Proof Mentorship Program.  PA Smith integrates both functional and conventional medicine to best serve his clients.  He has been practicing more that 18 years with over 100,000 patients cared for to date.   Please refer to bio section of the website.

Q. Is PA Smith an MD?

PA Smith is not a medical doctor but a physician assistant with a medical license to practice medicine in the state of Nevada.  He is certified under the National Commission Certifying Physician Assistants.  He is the owner and lead clinician for Holistic Hormone Solutions. PA Smith collaborates with Scott Jacobson, MD, family medicine trained and trained with the Institute of Functional Medicine.   PA Smith does not service medical emergencies, diagnose or issue protocols for any specific diseases. If you have a medical emergency, you must contact your primary care physician or dial 911.

Q. Can I email PA Smith with any questions/concerns?

Q. What is the time frame in which I should expect an answer to my email?

PA Smith and the office staff are committed to responding within 48 hours.

Q. What should I do if I am struggling sticking to my protocol?

We completely understand that many times, prioritizing a wellness plan requires major life changes. Our advice is to start one product at a time and take one day at a time. Often keeping a health journal allows one to stay on track and also shows the progression that we often forget when we need a little inspiration. Our lifestyle / holistic coaches are a great resource for encouragement and helping your through a difficult moment with your program.

Communication Outside of Scheduled Consultations

Many people now use email as a primary way to communicate with others. We appreciate that email can be a great way to ask a quick question or clarify something from your last visit, or share with us how you are doing. We have found through experience, however, that email is often not the best way to deal with more treatment-oriented questions and decisions such as questions regarding your medical issues, changes in your symptoms, or complex requests. Instead, in these cases please schedule an appointment so PA Smith will have time set aside to directly hear and address your concerns. If you have sent us an email and have not heard back from us after several days, please follow up with a phone call. Holistic Hormone Solutions, LLC maintains e-mail lists. To keep our customers and members informed. Individuals must affirmatively request to join our mailing lists. We do not sell, rent, loan, trade, or lease the addresses on our list to anyone.
